Purpose
This Privacy Policy describes the policies and procedures on the collection, use, and disclosure of your information when you use the website and Registered Dietitian services at Flourish Nutrition.
We may collect both personal and non-personal data in variety of ways, and use it to provide and improve services. Users may be asked about personal characteristics, health, activities, and views. Users can decline to supply personal data and visit the website anonymously.
For the purpose of this Privacy Policy:
Personal Data is any information that relates to an identifiable individual.
Service(s) refers to the website and Registered Dietitian (RD) services.
Service Provider refers to any natural or legal person who processes the data on behalf of the company. It refers to third-party companies or individuals employed by the company to facilitate the service, provide the service on behalf of thecompany, or perform services related to the service.
Account means a unique account created for you to access the service.
Company refers to Flourish Nutrition.
Cookies are small files that are placed on your device (computer, cellphone, digital tablet) accessing the website, containing details of your browsing history on this website, among its many uses.
Third-Party Social Media Service refers to any website or social network website through which a user can log in or create an account to use the service.
Usage Data refers to the data collect automatically, either generated by the use of the service or from service infrastructure itself.
Website refers to Flourish Nutrition, accessible from www.flourishnutrition.ca
You refers to the individual accessing the service, or the company, or other legal entity on behalf of which such individual is accessing or using the service, as applicable.
Who We Are
The company, Flourish Nutrition, is founded and run by Eva, Registered Dietitian. In the course of providing services, a number of consultants and agencies may have limited access to personal information held by the company. These include website consultants, accountants, and temporary workers to cover holidays, credit card companies, and lawyers. We restrict their access to any personal information we hold as much as reasonably possible, and have their assurance that they follow appropriate privacy principles.
Information Collected
Personal Information- Clients
Personal information is information about an identifiable individual. Personal information may include personal characteristics (gender, age, income, home address, phone number, email address, ethnic background, family status), health (health history, health conditions, health services received) or activities and views (religion, politics, opinions expressed by an individual). Personal information is contrasted with business information, which is not protected by privacy legislation.
Like with all Registered Dietitian services, personal health information is collected, used, and disclosed in order to provide dietetic services. We collect information about your health and diet history, your physical condition and function, and your social situation in order to help us assess what your needs are, to advise you of your options, and then to provide the health care you choose to have. A second primary purpose is to obtain a baseline of health and social information, so that in providing ongoing health services, we can identify changes that are occurring over time. It would be rare to collect such information without your informed consent, but this may occur in an emergency situation, or where we believe you would consent if asked and it is impractical to obtain consent. Information is stored in a secure and encrypted online practice management system to protect your privacy.
Health records are maintained for a minimum of ten (10) years after the last contact, as required by the College of Dietitians of Ontario. This information is also important when providing ongoing services over a period of months or years. After ten (10) years, electronic health records are destroyed.
With only a few exceptions, you have a right to review your health record for accuracy and currency. Often, all you have to do is ask. We will also try to help you understand any information you may not understand. We will need to confirm your identify before providing you with this access.
While using the website, we may ask you to provide personal information that can be used to contact or identify you.
Personal Information- Contract Staff, Volunteers
For individuals contracted to do work for the company, the primary purpose of collecting personal information is to ensure we can contact you in the future and for necessary work-related communication. Personal information may include personal characteristics (home address, phone number, email address).
Non-Personal Information
Like most companies, we collect, use, and disclose information for purposes related to our primary and secondary purposes. Some examples of secondary services include:
- To ensure high-quality services, including assessing performance of staff.
- To ensure compliance with Registered Dietitian regulations, by allowing the College of Dietitians of Ontario the ability to review records and interview staff as part of their regulatory activities.
- To report serious misconduct, incompetence, or incapacity of other practitioners.
- To report information suggesting serious illegal behaviour to authorities.
- To grant access to various government agencies (Canada Customs and Revenue Agency, Information and Privacy Commissioner, Human Rights Commission) which have authority to review records and interview staff as part of their mandates.
- To provide information to third parties (private or employer health insurance) which have your consent or authority to determine eligibility for funding.
You can choose not to be a part of some of these related or secondary purposes. However, there may not be a choice for some of these secondary purposes (e.g. external regulation).
Some of your usage data is automatically collected when using the service (e.g. device IP address, browser type and version, the page of our services that you visit, the time and date of your visit, the time spent on those pages, etc.). When you use the service through a mobile device, similar information may be collected (e.g. type of device, your mobile device unique ID, IP address, mobility operating system, type of mobile Internet browser, unique device identifiers, etc.)
The website allows you to create an account and log in to use the following Third-Party Social Media Services: Instagram. If you decide to register through a Third-Party Social Media Service’s account, we may be able to view some of your personal information (e.g. name, email address activities, contact list). You may also have the option of sharing additional information with the Company through your Third-Party Social Media Services account.
Cookies
Cookies are small pieces of information that are stored by your browser. We may use Cookies and similar tracking technologies (persistent cookies, session cookies, web beacons) to track and store the activity on our service. Cookies may be used to make improvement and updates, or improve the user experience. The company does not use cookies to collect and distribute information to third parties. A user has the option to not accept cookies.

Protecting Personal Information

- Electronic hardware is under supervision or secured in a locked or restricted area.
- Electronic information is transmitted through a direct line or email.
- Paper information is transmitted through sealed, addressed envelopes.
- Staff is trained to collect, use, and disclose personal information only as necessary to fulfill their duties and in accordance with the Privacy Policy.
- External agencies with access to information must enter into a privacy agreement with us.
Data transmission over the Internet is not 100% secure, and there are risks with transmitting information via online services. As a result, we cannot guarantee the security of the information you transmit.
Consent
This policy and all legal obligations of Flourish Nutrition are subject to the laws of Ontario and Canada, regardless of the location of the client. Any claims or complaints must be filed in the province of Ontario.
By using the service, you agree to the collection and use of information in accordance with this Privacy Policy. If you do not agree with this Privacy Policy, please refrain from using services.
We may update the Privacy Policy from time to time. You are encouraged to periodically review the Privacy Policy for any changes. Changes to this Privacy Policy are effective when they are posted on this page. Your continued use of the website following any changes to this Privacy Policy will be deemed your acceptance of those changes.
